The completed application form is the first step in
participating in the criminal background check process. To
become a member of veriFYI,
please download the application form and follow the
instructions. There are guidelines required by the State
of Texas that must be followed in order to be approved for this
program. Please note that turning in an application
form does not automatically qualify you for
membership.

Agency Service Agreement
Next, fill out the Agency Service Agreement. This form is
the agreement between your organization and the Volunteer Center
that stipulates that you will follow the rules for submission of
data, honor the privacy of the applicant, etc.

Background Verification Release Form
Before a name can be submitted through veriFYI,
a signed release must be completed by the applicant. This
step is required to legally obtain personal information on a
person. Your establishment may already have a form for
this purpose, however, the one provided here will include all of
the data fields necessary for data submission on the veriFYI
website.
